The recipient signs in to their own Discord account and opens the Nitro or gift card redemption page in User Settings.

Enter the card number and redemption code in the redemption field and submit.

Check that Nitro is active and shows the corresponding subscription period.

If the page asks for payment-method confirmation, follow the prompts shown in Discord.
